IMMIGRATION MEDICAL EXAMINATION  (I-693 FORM)

QUALIFIED CLINIC

The doctor is a Civil Surgeon for the United States Citizenship and Immigration Services.


FEES

EXAM WITH I-693 FORM:  $250

TESTS: Tuberculosis $190, Syphilis $23, Gonorrhea $64, Blood Draw $15

VACCINATIONS: TDaP $60, MMR $109, VAR (if no history of vaccination or disease) $181, flu (if season 1 October - 31 March), COVID x 3 including a bivalent booster (pharmacy $0), HepB (if <age60, new CDC recommendation) $107, PCV15 or PCV20 $327 (if asthma or age >=age65 and no history of any pneumococcal vaccination, new CDC recommendation)


ACCOUTING

We charge upfront. If you have Insurance which is a PPO health plan, we will bill them then reimburse you what the health plan pays us.  We always provide receipts, including for employer reimbursement and tax deduction.  If you wish to buy health insurance see our Insurance tab.  To immigrate to the United States of America, this is a one time health investment.

FEDERAL IDENTIFICATION: (passport, USA Employment Authorization Card with alien # (if any), USCIS online # (if any)

TIMELINE

The Form I-693 must be dated no earlier than 60 days before you filed your underlying application.  A properly and timely completed Form I-693 remains valid for two years from the date of the civil surgeon’s signature.


WHAT TO DO IF YOUR TUBERCULOSIS TEST IS ABNORMAL

2 billion people in the world have Tuberculosis infection.  If your tuberculosis blood test is abnormal, you will need to get a chest X-ray.  If the chest X-ray shows active tuberculosis then you will need multiple medications for multiple years.  If your chest X-ray is normal, you have latent (inactive) tuberculosis with a 15% chance it could activate so you will need one medication for 4 months.  As long as you get proper treatment, latent tuberculosis does not disqualify or delay your application.  Active tuberculosis may apply for a waiver.


FORMS

You do not need to bring a copy of the I-693 form.  We have the latest electronic copy (expires 31 Jul 2025 however see TIMELINE above).  Upon completion of all tests and vaccinations, we will ask you to return for a follow-up visit to review and sign your form with the doctor.  You will be given the completed form in a sealed envelope (do not open) for you to submit to USCIS.  You will also be given a copy of the form for your records.
